Overview:
We are seeking a skilled and detail-oriented MDS Coordinator to join our team. As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments for our patients. This position requires strong knowledge of medical coding, utilization review, and medical management.

Duties:
- Conduct comprehensive assessments of patients' medical conditions and care needs
- Collect and analyze patient data to determine appropriate care plans
- Ensure accurate and timely completion of MDS assessments in compliance with regulatory standards
- Collaborate with interdisciplinary teams to develop individualized care plans
- Review medical records and documentation to ensure accuracy and completeness
- Provide guidance and education to staff on MDS coding and documentation requirements
- Participate in quality improvement initiatives to enhance patient care outcomes
- Stay updated on industry standards and regulations related to MDS assessments

Requirements:
- Valid nursing license (RN or LPN)
- Proven experience as an MDS Coordinator or similar role
- Strong knowledge of ICD-9 coding and utilization review processes
- Familiarity with primary care, home care, acute care, and medical management practices
- Excellent attention to detail and organizational skills
- Ability to work independently and collaboratively in a team environment
- Proficient computer skills, including experience with electronic health records (EHR) systems

The expected pay for this position is between $36-45 per hour

Pay: $38.00 - $45.00 per hour
